Our Organization
During the summer of 2002, two public consultations designed
to gather input about community health and wellness showed
the extent of the community support. A volunteer group consisting
of community members, councilors, and health experts formed
teams around seven determinants of health:

Each team was charged with identifying and measuring a picture
of the town's health with respect to their determinant. Evidence
from the literature, and local and regional data, expert opinion
and experience, was gathered over a period of approximately
seven months. Partnerships were established with Niagara Regional
Public Health and Social Services, and a communication link
was established with the administration of NHS, which resulted
in a free-flow of information and support by the NHS of the
Community Health & Wellness efforts. Furthermore, Brock
University was engaged to work closely with us in the assessment
of Fort Erie's social capital and delivery of our Health Status
Survey.
Community/team member forums were held in April 2003, to:
a) review the Social Capital and Health Status survey results
b) review data/recommendations from each team
c) determine priorities for our action plan
d) determine the organizational/administrative model to enable
us to move forward
e) make recommendations to Fort Erie Town Council regarding
the organizational model of choice.
